• Right click the page to open Metform widgets
• Click New, leave blank selected then click Edit Form button
• Click ëAdd Templateí grey folder icon.
• Click ëMy Templatesí tab.
• Choose your imported metform template, then click ëInsertí.
• Once the template is loaded, click ëUpdateí.
